Spell damage amplification is for, who would have thought, spell damage only.
Any damage instance not dealt by your attacks is spell damage. Maelstrom's damage is not from your attacks, it's from the lightning jumping around.
The damage of bashes is attack damage. It's a raw attack damage bonus for that one attack that procs it.
The damage type is completely irrelevant, spell damage can be magical, physical and pure. Same for attack damage, it can be physical, magical and pure. Pure attack damage is not use currently in Dota, but magical attack damage is.

these are the heroes that have passives whos damage scales with spell amp

It works for pretty much all cast-able skills and spells, regardless of the damage type. Passives and on hit effects don't get the bonus damage as far as I can tell. You can check by holding ALT and hover over your icons, the only case I know of that it doesn't work but shows that it does is techies, and I believe that is because the mines are separate entities so the magic amp doesn't affect them.
Side note, I love picking it up on bristle because the damage stacks up super quickly, and that is a physical damage spell.
EDIT. I just tested, it does not amp up the damage, it still does 9% instead of the displayed damage
